- The Nurse Case Manager is responsible for coordinating and closely monitoring rehabilitation programs, communicating with internal and external treatment teams, coordinating care plans, ensuring clients and families receive necessary interdisciplinary treatment services, and writing progress reports.

- Strong interpersonal and analytical skills
- Current Michigan licensure
- 1-3 years of related work experience
