Nursing and home health aides
Rainbow Home Care offers the opportunity for clients to live safely and comfortably in their own homes, enjoying the company of family and friends.
The following professionals manage each person’s case and provide care:
A rehabilitation nurse is a valuable member of every client’s treatment team. After completing an initial assessment, the nurse will develop and manage a care plan tailored to the client’s needs.
The nurse assures that home health aides are fully-trained, coordinates care with rehabilitation providers, and—with permission—communicates with the doctor.
A member of Rainbow’s nursing staff and your program manager are on-call 24 hours a day for emergencies.
For those requiring assistance or supervision in their home, the primary caregivers will be home health aides who are trained in caring for individuals with special needs.
These specialized health care providers can assist with basic activities of daily living such as personal hygiene, grooming, dressing and meals. Home health aides also provide physical assistance with transfers, medical care and follow through with other needed services.
Like all Rainbow employees, home health aides treat every client with dignity and respect—the basic foundation of care at every level of our organization.
