Project Description

Personal Health Information Systems and Tools

Having a system or tool to record your personal health information helps ensure that you and your health care provider have access to current and relevant information when it is needed. This means that if you are seen in multiple different settings, communication and care are improved. There are many different ways to store your information; notebooks, medical id bracelets, flash drives, etc. Below is a partial list of the many different companies that can help you organize your personal health information. Rainbow can help! Ask your case manager if you have questions.
